The form below is ONLY to be used to submit your 2020 funding. Please note the following:

  • The information submitted below will be used to finalize your actuarial certifications and annual valuation reports. Please carefully review your entries and make sure they are accurate.
  • Use this form ONLY when you have made your entire 2020 contribution(s).
  • Remember that you have until the date you file your tax return (including extensions) to make your contribution(s).
  • At the end of the form, you will have a space to make any comments or ask questions.
  • A copy of your submission will be emailed to you for your records.
